1.3.2 Case Closure

Introduction

This procedure explains what is required in order to process the closure of a Child's record in the MOSAIC in instances where there is no transfer to the Common Assessment Framework.

1.1

The Lead Social Worker will ensure that the:

  • Parents/carers have been seen and spoken to by a social care practitioner within the 7 days of notifying the Team Manager that the child's record is ready for closure. A record of the visit will be recorded in case notes using title 'closure visit' and will record the carer/parents view of the closure;
  • The child/young person has been seen and spoken to alone by a social care practitioner within the 7 days of notifying the Team Manager that the child's record is ready for closure. A record of the visit will be recorded in case notes using the title 'closure visit' and record the child's view of the closure;
  • The accommodation which the child/young person is living in has been seen by one of the professionals involved in the provision of a service listed in the child's plan. A record of the visit will be recorded in the child's record in case notes using the title 'accommodation seen'. Within two working days of the visit the professional visiting will provide a written or verbal record of the visit to social care for it to be recorded in the case notes;
  • The Chronology, core person details and all areas of the child's record are up to date.
1.2 The Lead Social Worker will notify the Team Manager that the child's record (referral) is ready for closure. A closure discussion may take place and will be recorded in case notes.
1.3 The Team Manager will read the child's record within 5 working days of the notification and record on Case Note discussion/decision of whether the closure is agreed or if further action is required.
1.4 The Team Manager will check that there is evidence on the child's record that the Lead Social Worker has completed the actions above.
1.5 Following the Team Manager confirming closure agreement within 5 working days the Lead Social Worker will complete all the sections of the child's MOSAIC record required to close the referral following the steps set out below:

Lead Social Worker role. All services that a child has been receiving need to have been ended before the Referral can be closed. Services must have an end date and reason recorded against them as follows:

  • Service with actual and planned end date, provision end reason and any changes to the service notes (this can be used to record that the service will continue using the CAF);
  • Needs with the no longer required date, reason for no longer required, outcome options and status;
  • Child's plan - select completed, date of completion and reason for the end;
  • End involvements, end Lead Social Worker role and add Team Manager/ Principle Practitioner as Lead Social Worker.

The Team Manager/Principle Practitioner will read the closure visit record and will close the referral by completing:

  • Overall summary of progress achieved, reasons for successful outcomes or for non or partial achievement of planned outcomes;
  • Views of child/young person about closing the case;
  • Views of responsible adults about closing the case;
  • Has child/young person been provided with a copy of closure record?
  • if no, when will this be done?
  • Have parents/carers been provided with a copy of closure record?
  • If no, when will this be done?
  • Other action(s) (please specify);
  • Reason for referral being closed;
  • Date/time closed;
  • End Lead Social Worker and key team;
  • Any other requirements to close a referral as in the MOSAIC user guide.
1.8 The child, parent(s), carer(s) and other professionals involved will be provided with a copy of the closure record exemplar following the closure of the referral.
